24 HOUR CANCELLATION POLICY
In an effort to provide all patients with quality care in a timely manner, Axis Health Center has updated its appointment policies. As a courtesy to other patients, our doctors and providers, we ask that you read and understand our appointment policies.
MISSED APPOINTMENT / LATE CANCELLATIONS
Patients who fail to arrive for a scheduled appointment or do not notify our office of cancellation within at least 24 hours before their appointment time will be charged at the rate of 50% of the scheduled treatment, billed directly to them, and payable prior to their next visit. We understand that unexpected circumstances may arise which is why we provide both email and text message reminders prior to appointments. This allows patients ample opportunity to notify us prior to their appointment and to avoid missing an appointment and accruing late fees. If there is a credit card on file the charge will be put through and the receipt emailed to the email address on file. If there is no payment method on file the fee must be paid prior to the next scheduled appointment. If a patient is a no-show more than once, the patient may be required to prepay for any future appointments.
PAYMENT FOR MISSED APPOINTMENTS/LATE CANCELLED APPOINTMENTS
Missed appointment fees will be charged directly to patients. SGI, WCB and other insurance companies do not cover the cost of a missed appointment. It is insurance fraud to claim a receipt for a service that was not provided. For this reason, we cannot direct-bill for missed appointments. We can provide a payment receipt of the related charges.
LATE ARRIVALS
Our staff will do their best to accommodate patients if they are running late, though it is not always possible with the provider’s schedule. Our priority in these cases is to keep the doctor/therapist on schedule for those patients who have arrived on time for their appointments. If you are late we will try to fit you in, however this is not always possible.
REBOOKING NEW PATIENTS FOLLOWING A MISSED INITIAL APPOINTMENT
The above policy also applies to new patients. New patients who have rescheduled following a missed or late cancelled appointment will be required to prepay for the rescheduled visit.

INSURANCE POLICY
It is the sole responsibility of each patient to understand the details of their insurance policy and coverage for each provider type prior to receiving any treatment at Axis Health Center. The clinic is responsible for providing quality and timely services to patients. The clinic is not responsible for understanding the details of patients’ insurance coverage. Patients are responsible for paying for all services rendered regardless of their insurance coverage details.
DIRECT BILLING POLICY
We require a credit card (Visa, Mastercard, Visa Debit) on file when providing direct billing services. This is for security purposes if there is any remaining amount not covered from the insurance policy. Patients who choose to have us direct bill to their insurance company on their behalf are responsible for any remaining amount not covered under their insurance policy.
